Otrava was a member of the Sisterhood, a healer and one of Phosani's closest companions. When the kingdom of Hallowvale fell to Lord Drakan's forces in Third Age, Otrava along with Phosani and Insela swore allegiance to the Zamorak and the vampyres and handed over their remaining squadrons to the invaders. Otrava followed Phosani in founding the town, that would later be known as Slepe in east Morytania.
Using old texts from Senntisten, Otrava uncovered the secrets about blood magic and blood healing, which she had witnessed the Zamorakians do. She started experimenting with potions with blood and came up with the battlemage and bastion potion, which she documented in a book. There were two other potions that she discovered, but cancelled development of them, one of them being too dangerous and the other missing a "vital" ingredient.
Otrava's greatest achievement was the Elixir of Everlasting, a potion that would greatly extend the drinker's life and appeared to make them younger in the process. She tested it out on herself, before giving it to Phosani and the other elders. Although she noted some drowsiness at first, she shrugged it off as insignificant. The potion in turn allowed them to construct the Sisterhood Sanctuary, where Otrava noted that some of the sisters were sleeping far longer than usual, but she believed it was a result of hard work. Unfortunately, her failure to detect this as a serious symptom resulted in her and the others who drank it falling into a deep slumber, and they were placed inside the Sanctuary in coffins as a safety precaution. Otrava did set some safety measures in place however, such as creating false documents to prevent the formula from falling in the wrong hands. Before she fell into her slumber, anyone that wanted to make the Elixir of Everlasting had to do so under her supervision.
Otrava wrote Bloody notes which is a diary about her encounter with talking tree in south Humblethorn forest in early Third age. She wrote down powerful magic spell on a runescroll which when read will unlock ability to craft bloodbark armour.
