Care map

A care map describes the steps and decision points in the management of a condition. It is based on medical guidelines, recent evidence, and expert consensus. A care map is made up of one or more pages which together show the complete patient journey for a condition.

Characteristics

A care map page consists of care points that are connected together as a series of options and recommendations to show how individual care decisions are related to the patient journey. A care map that has been customised to represent how services are delivered within a particular health economy is called a local care map.
